Exclusions

  • Any medical condition or complication arising from it which existed before the commencement of the Policy Period, or for which care, treatment or advice was sought, recommended by or received from a Physician.
  • Routine physical or other examination where there is no objective indication of impairment of normal health.
  • Medical expenses beyond the expiry of the Policy Period.
  • Suicide, attempted suicide or wilfully self-inflicted injury or Illness, mental disorder, anxiety/stress / depression/nervousness having no underlying physical illness as a cause; venereal disease, alcoholism, drunkenness or the abuse of drugs.
  • Manual work or hazardous occupation, self-exposure to needless peril (except in an attempt to save human life), or if engaging in any criminal or illegal act.
  • Pregnancy, resulting in childbirth, miscarriage, abortion, or complication arising out of any of the foregoing.
  • Experimental, unproven or non-standard treatment.
  • Treatment by any other system other than modern medicine (also known as Allopathy).
  • The cost of spectacles, contact lenses, and hearing aids, crutches, and all other external appliances and/or devices whether for diagnosis or treatment.
  • Delay of baggage when the intended destination is in India
  • Loss or damage to the Insured's passport as a result of the confiscation or detention by customs, police or any other authority.
  • Loss which is not reported to the appropriate police authority within 24 hours of the discovery of the loss, and in respect of which an official report has not been obtained.
  • Loss caused by the Insured's failure to take reasonable steps to guard against the loss of the passport.

It is important to notify Bajaj Allianz immediately before seeking Medical Assistance. Use the 24x7 assistance numbers:
For Hospitalisation
If you are admitted to a hospital after an OPD consultation or otherwise please notify Bajaj Allianz immediately. The Insurance medical assistance department (doctor) will discuss your medical condition with the Treating doctor and if it is confirmed that the admission to a hospital is NOT due to any pre-existing conditions or any conditions listed in the exclusion list, then the Insurance Company shall guarantee payments to the hospital and settle the payments with the hospital.
Documents Required:
In order to expedite the processing of the claim you must send the following documents immediately by fax to +91-20-30512207 or scan and email to us at travel@bajajallianz.co.in
  • Completed Claim Form signed by the Treating Doctor and You
  • Name, address, and contact details of the Local / Family Medical Doctor in India
  • Signed copy of the RELEASE OF MEDICAL INFORMATION FORM (ROMIF) to authorize your Treating Doctor and Local Medical Doctor in India to release your medical information to the Insurance Medical Assistance Department
  • Treating Doctor's medical report
  • Estimated medical expenses
  • Date of Travel from India
  • Details of domestic health Insurance policy, if any (Policy No, name and address of insurance company)
  • Copy of passport with an exit stamp from India and entry into the overseas country
 
For Outpatient Treatment
For outpatient consultation, you will have to self-pay and file the claim upon return to India. However, if the amount is above USD 500 and you are facing difficulty in making payments due to a shortage of cash on hand then Bajaj Allianz can arrange to make payments on your behalf of you, provided the medical condition is confirmed to be non-pre-existing.
Documents Required:
The important documents required to file a claim are as follows:
  • Claim Form (To be signed by the Treating Doctor and Insured) and with the Claims Reference Number.
  • Doctor's medical report.
  • Original admission/discharge card
  • Original bills, receipts, and prescriptions
  • Original X-ray, pathological and investigative reports
  • Copy of passport, visa with entry and exit stamp

  • Details of compensation received from Airlines/other authorities
  • Bills and receipts of purchase of emergency toiletries and medicines
  • Obtain confirmation of the delay from the airline.
  • Claim Form can be obtained from Assistance Department
  • Fill in the Claim Form and submit all documents to the address mentioned on the policy schedule
Delay of Baggage

  • Details of expenses incurred in obtaining a fresh passport
  • File a complaint with the local police
  • Contact the Indian Embassy, wherever necessary
  • Submit all documents to the address mentioned on the policy schedule
Tuition Fees
  • Duly filled in the claim form
  • Copy of the receipt of payment of advance tuition fee
  • Letter informing the school regarding the inability of the student to continue the semester duly acknowledged by the school.
  • Discharge card of hospitalization along with detailed medical report and all investigation reports.
  • Obtain confirmation of inability to attend school from the school authorities
  • The claim form can be obtained from our Assistance Department
  • Fill in the Claim form and submit all documents to the address mentioned in the policy schedule
Accident to Sponsor
  • Duly filled out personal accident claim form.
  • Copy of the death certificate of the deceased
  • Death Summary from the hospital where last treated.
  • Letter from the school authorities intimating about the balance tuition fee if any for the course.
  • Any other documents if required will be informed by the claims department of Bajaj Allianz
  • Inform Bajaj Allianz Assistance Department regarding the incident
  • Submit all documents to the address mentioned in the policy schedule
Family Visit
  • Duly filled Medical Sickness or Personal Accident claim form
  • Certificate from the doctor treating you mentioning the requirement of an attendant & reasons for the same
  • Travel tickets in original along with a copy of the visa & passport
  • Inform Bajaj Allianz Assistance Department regarding your condition
  • Obtain prior authorization that the travel of the family member will be covered
  • Submit all documents to the address mentioned on the policy schedule
Personal Liability
  • The full statement of the facts in writing
  • Witness statements
  • Any other documents relevant to the incident, including Summons, Legal Notice, etc
  • Inform our Claims Dept. immediately (at the address given on the policy schedule below) giving full details of the incident.
  • Do not commit any benefit/compensation or enter into an agreement.
Accidental Death & Disability
  • Bajaj Allianz Claims Dept. will advise on documentation upon receipt of claim notification
  • Collect all documents pertaining to the loss including correspondence with the carrier and submit all documents to the address mentioned on the policy schedule
  • Claim Form can be obtained from our Assistance Department
